Don’t see your payer?

Check the remittance advice or EOB for the paying entity’s name, many plans pay through a clearinghouse (Zelis, ECHO, InstaMed) rather than under their own name. If it’s still missing, start from Enroll in EFT and ERA, which covers how to find any payer’s enrollment path from scratch.
