The four doors
There’s no single place to enroll. The payer directory says which door each payer uses. Aggregators default many payers to virtual card and bury the ACH election in settings or a fee-bearing “premium” tier. Choose standard ACH. It must be offered without percentage fees.The recipe
- Build the payer list from money, not memory. Rank 12 months of deposits by dollars. Don’t miss quarterly payers, workers’ comp, and deposits descriptored ECHO/ZELIS/OPTUM.
- Gather the packet once: legal name, TIN, Type 2 NPI, credentialed address, routing/account, voided check or bank letter, authorized signer.
- Work top-down by dollars, batched by door. Per payer: EFT to the designated operating account, ERA to your clearinghouse (confirm the receiver ID with your PM vendor), and record the stated processing time.
- Elect ACH wherever a vendor offers VCC. If a payer resists, cite 45 C.F.R. § 162.1602 and escalate to provider relations. It works.
- Track to verified, not submitted. Done means: a real claim paid by ACH into the right account, its 835 arrived and posted, the two reassociated. Processing runs from days (Optum Financial: about 5) to 4+ weeks. Some verify by penny deposits, some by a call to your admin (EnrollSafe).
Maintenance
Enrollments break silently: bank changes (full playbook), address or TIN changes, annual re-verifications, and payers migrating between payment vendors (which can silently reset you to VCC or check). The tell is a payer’s deposits stopping or changing form.
