Enroll in EFT or update your bank account
These are sequential, each one depends on the last:- Complete Form CMS-588 (EFT Authorization Agreement) to add or change your EFT setup.
- Attach a preprinted voided check or a bank letter showing the provider name and banking information.
- Have the authorized representative or delegated official sign the form.
- Submit to Palmetto GBA Part A Provider Enrollment, 2300 Springdale Drive, Building One, Camden, SC 29020 (or via PECOS).

If you are switching banks
Re-run the enrollment above with the new account details. Until the change is confirmed, payments continue to the old account, keep it open and sweep the balance until deposits from Palmetto GBA (Medicare MAC) appear in the new account.
